VBA Excel 2007 Date Picker keeps uninstalling - Help?

Hi all,
I need some help with an action tracker I made i needed it as locked down and idiot proof as i could so i made the attached one.

Its setup so that when you click add action it will come up with a user form to guide the user through the required things to add the action, then it adds it to excel and relocks.

the issue im having is randomly - maybe daily(?) the date picker in userform1 deletes its self for no reason, then i get a complie error when opening the userform.
ive fixed it in the attached one and i cant get it to break again manually but does anyone have any ideas why it keeps removing its self?

A couple of side notes
when i got into VB i notice that the toolbox with all the controls is gone - dunno if that means anything?
all people using it are on excel 2007. some are win 7 but most are XP.
I started making a userform3 with a calendar control, it doesnt do anything but that also goes when i have this issue...

any suggestions? any way round it?

the VB password is vmax
