Insert New row in another sheet and Loop based on values in certain cells

Dear Friends,

I am designing a entry screen for some products.

I need to make like a FORM to enter the data and accordingly a ROW should be inserted in another sheet (LOOP).

I have attached a sample file for your kind reference.

I will be highly obliged if helped. Please help this novice....
